Villain is a nanobot swarm with consciousness. Here is my question.
Does Injury Tolerance (Diffuse) impart the ability to go through grates or under around door seams etc? |

No. It prevents you from being hurt, but doesn't let you get through narrow spaces. For that you want one or another of Slipperiness (to squeeze through narrow openings), Shadow Form (to squeeze through incredibly narrow openings), or Permeation (to pass through porous materials or substances). Take a look at those three and see which one best fits your character concept.

Well, you could get Injury Tolerance (Diffuse) with the Infiltration enhancement from Powers pg.53 for +40% to the cost which does allow you to get through narrow cracks and porous barriers.

I had forgotten about that one—Powers isn't immediately at hand; I just reshelved all my GURPS books—but yes, it's a viable approach.

Invertebrate might also help. "Much smaller openings than your size would suggest" isn't all that precise to start with, but even more so for a Diffuse creature.
