It was a Deadlands game. We had got trapped in a village in some sort of... pocket dimension or something (basically if we walked in any direction we just ended up coming back to the village). There was some sort of monster terrifying the village, and we knew the children were involved somehow. After some (honestly quite frustrating) investigation a fight broke out, and in response a "rainbow dinosaur" burst round from one of the buildings. It was described as looking like a kid's drawing, and it turned out that the children's imaginations was terrorizing the town and this was a child's picture come to life.

I think the episode of Twilight Zone (or was it The Outer Limits?) was the first one, at least on television.
