Disadvantage: Hydrophobia
I have rabies. I plan to stack Infectious Attack, Terminally Ill, and Berserk (always on). Maybe some social disadvantages as well. Did I miss anything?

Are you human? If so then Berserk isn't realistic. Humans really just get twitchy and convulse.

Violence while uncommon isn't unheard of.

I'm given to understand there's a bit of throat discomfort. Perhaps some level of Chronic Pain?

As far as I know only human being has ever survived it without brain damage, and she was a child put in an induced coma.

Well, I wouldn't have a clue about the breaking of one's own bones. But the convulsions could be covered by Epilepsy. So take the highest level of pain, and make that and Epilepsy Symptoms maybe? In addition to whatever of the above the OP decides on. Incidentally, those huge penalties to attributes from the worst pain could be said to cause broken limbs when they cause a critical failure.

But certainly the neurological effects are very severe - large reductions in DX and IQ at least, and by tomorrow effectively unconsciousness and paralysis. But that's boring - it doesn't make for a playable monster.

Blast, the problem with aging no one talks about. The information you learned "just yesterday" turns out to be decades out of date.
I don't know. Just extend the violent incoherence to a few days and you still can get a reasonable horror infected type zombie. It might lend a verisimilitude that completely made up diseases might not.

Re: Disadvantage: Hydrophobia
I've always been keen on the idea that rabies could feasibly mutate such that it takes control of neurological functions and through that, allows sustained animation... for the purpose of gaming anyway.
