Depending on your tech level/assumptions it might just be easier to craft a 'custom' body tailored to those needs and either do some sort of consciousness transfer or similar.

Yeah as mentioned above head-shrinking seems like a real chore. Which bits of the brain are we going to plane off? Even when it could be done safely, people might still be unreasonably squeamish about a partial brain-ectomy.

Seems likely that the state of the art would stay at some version of a "cut long bones and fuse" procedure up until the TL allows custom body swaps.
