Matrix, you have given me the answer! On MA.109 that you refernce, the bullet point of attacks with limited reach. It spicificly says:

Attacks with limited reach. If a Combination involves an attack with fixed reach, that attack can only work if the
preceding ones leave the target in range. In particular, if a closecombat strike like a punch or a Knee Strike follows
an attack that hurls the victim one or more yards away – most
often a shove, throw, or strike that inflicts knockback – the
Combination ends if the attacker can’t step to deliver the
short-ranged attack.


That should apply here.

Also, the paragraph you pointed out does say that knockback would take the target out of range.

Check Basic pg 111. "An attack that inflicts crushing or cutting damage normally inflicts knockback... Add this limitation if it does not." What's discussed in Powers are suggested builds, not the only way things can be made.

And yes, 20 damage to a 10 HP target is likely crippling or lethal... unless that target has DR. If the Hulk is hammering on Iron Man with his fists, he'll want some way to keep him in place long enough to actually penetrate the suit. Varying the "direction" of knockback can do this, as well as allow cinematic stunts like the juggling blows seen in some video games and movies.
