I've done those other things dozens to hundreds of times but only once hurt my back.
Once you feel the need to require sequential critical failures to model something properly, I think it should just be ignored in game.

I mean, when we have critical hits, we roll on a table, and the most common results are "normal damage". I don't see why a critical failure wouldn't be pretty much a similar thing. I'm sure we probably make plenty of critical failures through-out the course of a week, or even a day, we only notice the ones that cause us real discomfort.

I always see the Bad Back injury as being caused by severe physical injury, usually from injuries to the muscles anchored to the spine and/or shoulder, though neurological injuries might cause a Bad Back injury. I would suggest that a major wound to the back of the torso would probably cause Bad Back (Mild) until while a major wound to the spine would probably cause Bad Back (Severe) until healed.
A character should make a HT roll when the injury heals. On a success, the Bad Back fades. On a failure, the Bad Back would be permanent. On a critical failure, the character permanently aggravated the injury, turning Bad Back (Mild) to Bad Back (Severe) and Bad Back (Severe) to Bad Back (Severe) and Paraplegic. Physical therapy would give a bonus to the HT roll. |
