Based on the official examples, that's how Perks work. Alchohol Tolerance doesn't help you occasionally when you are drinking. It works EVERY time you drink.
Penetrating voice gives a +3 for anyone to hear when you want to be heard. some of the time? No. Always, or at the character's discretion. I cited Weapon Bond already.

Weapon Bond: +1 to (let's say) Broadsword skill when using one specific, unique sword. Not +2 to your Broadsword skill whenever you use the skill!
Penetrating Voice: +1 to your Intimidation skill only when you surprise someone by sneaking up on them and yelling at them. Not +2 to your Intimidation skill whenever you decide to use the skill. Bonuses from Perks are for specific uses of the skill in rare or difficult to achieve situations. This is why Weapon Bond does not say, "You have +2 to one weapon skill, but only when you're using it to wield a weapon," because that's a crock. That's like buying Insubstantiality and taking the limitation, "Accessibility, Only to become Insubstantial". If you want +2 to your Intimidation skill for all uses of the skill, then you raise your skill by 2 levels. You don't justify a +2 universal bonus to the skill in a 1-point Perk. Or maybe you do in your game. Heck, why stop there? New Perks: Sexy Face: +2 to Sex Appeal skill, only when you're trying to make someone attracted to you. Axe-Throwing Arm: +2 to Thrown Weapon (Axe/Mace), only when you're throwing an axe or mace at a target. Adjusted to the Saddle: +2 to Riding (Horse), only when actually riding a horse. etc.

Resisting drinking isn't a skill, it's a rare use of a HT roll. A +3 to being heard is NOT an always on + to other people's hearing rolls, just when you're yelling over a crowd. These are all fairly rare circumstances. Fur for example allows an "always on" immunity to sunburn. Handy, always on, but rare. It's the same as saying he always makes hit HT roll to resist a rare, vaugely annoying condition. All of those are much, much less useful than a +2 to intimidation whenever I decide to do it face to face.

The circumstances are supposed to be commensurately less common, the greater the bonus. Frinst: Weapon Bond is not a +1 to <weapon skill>, but a +1 effective skill to one specific weapon only; the bonus from Penetrating Voice does not apply to a skill, but to a particularly uncommon situation, and so on.

Maybe the Intimidating Appearance could be balanced by looking at the negative effects - you look intimidating, so people will tend not to relax in your presence, confide in you, etc... (-2 to reaction rolls in those circumstances? -2 to Diplomacy?)
