Less than lame, lower level crippled limbs.
Personally I like including the old soldier archetype in my games. Sometimes I want them to be capable fighters with many years of experience and the scars to prove it. However the long term damage disadvantages all seem a little too drastic.
Has anyone played with lower levels of permanent damage? I have used the quirk "distinctive gait" to cover a limp. I wrote up "bad temper, mitigated by pain killers (or alchohol)" for long term pain. On the positive side there is the weather knee perk "knee is aching, a storm mus' be comin'" Edit Trick shoulder/knee after using extra effort there is a chance that it seizes up.
