You're slightly off, +11 is converted to 3 dice.
There is no official chart, but it's not that difficult to deduce it from p. B269.

On the same topic, but in a different vein: from what my gaming friends tell me, GCA (GURPS Character Assistant) converts 1d+3 to 2d-1. From the optional rule on p. B269 I can't see anything about it, only that +4 = d and +7 = 2d etc.

Which is correct; 1d+3 stays as it is or converts to 2d-1?

Using RAW, 1d+3 stays 1d+3.

However! Your friends are wrong about GCA. GCA converts +7 to 2d, +4 to 1d. It does not, as shipped, convert +3 to 1d-1.

(It is possible to *change* GCA to do the conversion as you note, but it is not RAW, and it is *not* shipped capable of doing that.)

By default, GCA does not convert damage pluses into dice; if you turn on the option it will convert +4=+1d and +7=+2d, as per the official rules, unless you load an optional data file (such as the 4eaddon from my website) which breaks from the official progression. 4eaddon is set up to convert +3=+1d-1, +4=+1d, +5=+1d+1, +6=+2d-1, and +7=+2d, but that's even more unofficial than the optional dice conversion guidelines.

Oh, yeah, I'd forgotten to mention that the dice conversion wasn't even turned on by default.

And I suspect that your add-on file, or someone else's similar tinkering, is probably the source of the confusion for the friends in question here.
