Area Knowledge to know what you should be trying to do, Shiphandling to actually do it. Much of the Shiphandling would be "You can do that" or "Don't roll a critical failure" if your Area Knowledge is good, with actual Shiphandling rolls reserved for actively dangerous parts of the river.

The pilot tells the steersman what to do; the steersman does the shiphandling.

Navigation would only come into play if you don't know already which way to go on the river. Area Knowledge would tell you this if you have it for the area but if you've journeyed overland Navigation would let you figure out which way along the river you need to go to get there.

Shiphandling is the skill of captaining a ship. If you're telling the steersman what to do while they handle the ship, you're using Shiphandling, and they are fairly likely to be using Crewman.
