I don't mean to be crude. But are there rules for grappling testicles (as an attack)? What are they or what should they be?

I've been thinking about this too for some reason. You could probably do it with several Extra Arms (Long, Flexible, possibly Weak). Or else a Binding attack.

In respect to extra arms, does the weak limitation also reduce the effective HP of the limb, making it easier to cripple / dismember?

Yes. IDHMBWM, but there is a Testicle Grab technique published in the excellent Fairborne Close Combat Systems.
