Default Optional Bad Stuff or Death

I had a scenario recently: my friend was out of cards and lost all of his equipment. Could he choose to suffer Bad Stuff instead of trying to run away (in order to trigger death and thus get 8 cards on his next turn)? Is he able to kill himself in order to reset his hand? Or is he just stuck until he gets more cards somehow?
By the way, we ended up making him roll to run rather than letting him suffer the Bad Stuff (death in this case) and much to his dismay, he escaped.
