Default In-character character sheets?

A thought occurred that might be interesting, and I was wondering if there were any games that used this concept. That is, a character sheet intended to look like an in-character document, such as a resume, personnel file, dossier page, etc.

CASTLE FALKENSTEIN does something like that. It has no form for a Character Sheet, but instead encourages each player to keep a "journal" or "diary" for his character in which he puts background notes, sketches or swiped illustrations, and commentary on the events of each game. I never got quite that immersive in the Falkenstein campaigns I've run, but it would be cool to do that some time.

Top Secret:SI designed its character sheet as a personnel file for the Orion Foundation complete with space for psychological evaluation. To complete the verisimilitude, my sisters used to clip head shots from magazines and catalogs to attach to the "dossier."

I am at the moment hosting a game where the characters remember nothing about their past. You know, the classic.

For a couple of sessions now we all enjoyed writing down discovered character traits in "plain" words (some Turkish some English). We already have different illustrations for each character (3D renders done by me).

I am half inclined to never give them full character sheets but to let them describe stuff in their own words and in their own style.

Worked good so far...

MSPE's official character records look vaguely like personell folders.

Hollowpoint has one official sheet that looks like a mortician's toe-tag.

LUG-Trek TNG used a sheet that looked like the LCARS displays from TNG.
