Level Draining +5 Rules

Does the level draining buff on a monster cause the monster to come up continuously on the draw until someone defeats it, or is it a one time reoccurrence?

I believe it keeps coming up during every Kicking Open the Door phase until somebody can defeat it.

Since the Level-Draining card comes back with the Monster, the rules on that card would apply to every combat that Monster (re)appears in. So, basically, it will continually pop up on every player's turn until someone defeats it.

Does it come back alone, or should there be a new card flipped and therefore a possible team-up monster or monster-modifier for each successive player?
If a modifier comes up, should it also persist from turn to turn or should it be viewed as a 'passing phase' for the monster?
We had this situation where the first monster faced in the game lasted more than an hour before it went down, and there was much wailing and gnashing of teeth.

I'm fairly sure I remember a rule that it comes up in addition to whatever card you actually draw. Otherwise, there's virtually no way to get enough cards to actually beat the thing.

Any other modifier would remain in the discard pile after the combat it is played in. Only the monster and Level-Draining return.

Edit: Here's a post where MM makes it clear you still face a Door drawn from the deck in addition to the Level-Draining monster.
