I may be metaphorically blind, as I'm looking at that section and can't see what counts as a narrow passage that is Escape-able for a given SM, what counts on a pass-without-a-roll passage, and what counts as no-chance-even-with-a-roll passage.

It's not strictly SM-based, because SM isn't the only factor. Underground Adventures says "To get through [passages], a traveler can use Escape skill to squeeze through a passage only slightly wider than his body" (p. 12). So you have to determine how wide a creature is. An SM 0 humanoid might have a harder time squeezing through a 1.5-foot passage than an SM +2 snake would, because the snake's SM is from its length, rather than its width.
