I take it that they reflect almost diametrically opposed underlying processes. Impaling is something like an arrowhead, a spear point, or a dagger: it's designed to impact a very small width of skin and therefore to exert a high shear force, pushing flesh aside as it sinks in. Huge piercing is something like a very large modern bullet: It has a bit of a point, but what it's mainly doing is hitting with a lot of force. One's a wedge and the other's a cylinder with a rounded end.
