Lulu has a cover design wizard with which you can make your own personalized copies. I've made my own covers for several PDFs. The options are limited as far as font choice and lettering placement, but the cover art can be whatever image(s) you upload. I'll send you my cover images so you can judge for yourself if you give me your e-mail address.

Unfortunately I didn't like Lulu's cover designer very much. What I ended up doing for the covers of my DF Omnibus was just creating my own front and back cover images (combining the cover images of DF 1-4). I then just removed everything I could from Lulu's cover designer and uploaded my own cover image.

Editing the images? I just used Paint.Net, which is a free image editing suite for Windows. I believe I got screen caps for the four covers then pasted into Paint.net, then manipulated the black borders until things sat right. Final image size was 2500x3100 pixels. I've still got the cover images I used for Dungeon Fantasy; if you're interested please PM me.
Setting it up on Lulu? I don't quite recall exactly how I did it, but essentially I set the cover photo in Lulu to my cover images then removed all the extra text that Lulu adds. The cover designer template I used within Lulu was "ImageOnly" which is very minimalist to begin with.
