Default "Player on the right takes an item card from you" question

In some cards it says that the player on the right and/or the left may take an item card from you. I'd like to clarify if this item card is one that is in play (either equipped or unequipped), or one from the hand? Thanks.

If a card says that and only that, then it comes from those in play on the table.

Cards in hand are only cards, though they may have the potential to 'become' items by being played to the table and thus carried, right?

Yes, cards do have that potential, however once this effect is triggered, it must be resolved immediately with Items that are already carried. There is no time to play any cards from your hand to carry them before the effect resolves once it is triggered.
