Not fully healthy, but not fully 'dead' ones either, as far as I recall, to be replaced with better/healthier ones. That seems to correspond pretty closely to 'amputate a damaged but not destroyed limb, replace with a prosthetic'.
(I don't remember whether people ever destroyed 100% healthy immune systems to replace them with those compatible with
another transplanted organ. Maybe yes, maybe no.)
