Yes.
Harsh Realism is my go-to condition, both in
Tactical Shooting (p. 32) and
Martial Arts (p. 124). Regarding the unarmed attacks, my personal experience with Judo, Taekwondo, and Krav Maga is that your off hand most definitely is at a penalty. Yes, you can train that away (although if completely I don't know), but it should be the starting condition.
