People trained to do this probably operate at ST0 to do as little as possible so it is always the 1 min. Long as it isn't combat they probably have "no nuisance" to prevent critical misses and hits as long as patient isn't struggling.
Piercing, probably small piercing.

Also, not everything that breaks your skin should be modeled as an attack under combat rules.
