Default How big is a hex?

I couldn't find this in a previous post so I'll ask here. How big are hexes supposed to be. I get the sense from ITL that a hex is only a yard across, but that seems too small considering:

1. A wolf, a large bear and a dead body typically take up one hex. A horse (8 feet long) takes up two hexes.

2. Hand to hand (i.e., unarmed or knives) happens in one hex.

3. It seems it would be difficult for fighters with spears, swords, etc to fight only 3 feet away from each other. Whether with each other or side by side.

So hexes would seem to be more like 4-6 feet. Am I missing something?
