I'd say that potential fight-enders are assigned penalties in proportion to their worth and not their difficulty.

Yep, one of the first things they teach newcomers in boxing after how to throw a punch is how to cover up and defend. And they teach how to throw controlled punches and not over extend so that you can recover quickly from attacking and defend afterwards!
Most real life pub fights (or school fights) are really not much like fights in GURPS between trained fighters.
