If you don't mind my saying so, it really sounds like you guys need to get in one or more professional project managers to go over your processes and help you get on the right track. I've been at software companies that have bad processes, and a lot of what you describe - run and fix the current crisis is the order of the day - is what goes on.
As always, I'm really glad to see how things are going, and I hope you folks made a profit.
