I always see the Bad Back injury as being caused by severe physical injury, usually from injuries to the muscles anchored to the spine and/or shoulder, though neurological injuries might cause a Bad Back injury. I would suggest that a major wound to the back of the torso would probably cause Bad Back (Mild) until while a major wound to the spine would probably cause Bad Back (Severe) until healed.

A character should make a HT roll when the injury heals. On a success, the Bad Back fades. On a failure, the Bad Back would be permanent. On a critical failure, the character permanently aggravated the injury, turning Bad Back (Mild) to Bad Back (Severe) and Bad Back (Severe) to Bad Back (Severe) and Paraplegic. Physical therapy would give a bonus to the HT roll.
