Probably because it's an optional rule essentially hidden in medical rules in a book that mostly isn't about combat (
Bio-Tech). Personally, I consider that consequence over-deadly, and would allow a HT roll to avoid it in anything but the most gritty campaign. The extra injury from crushing attacks to the neck is already deadly enough.
