In Korea, there is an accident that a doctor who ordered his nurse to do endobronchial anesthesia is accused and his license is suspended for 3 months.

That's a matter of society rules and norms, not a direct representation of actual skills.
In GURPS terms it might be because society expect that the nurse has skill-level:12 where the doctor is expectet to have Skill-level:14.
However some experienced nurses likely have higher skill levels than rookie doctors (or even experienced doctors).
