Relative scaling might roughly correspond with size but I strongly suspect that Hit Penalties were originally based on gamist considerations. Reality checking from my long ago youth suggests that punching people in the face is nowhere near to being a -5.
I'd say that potential fight-enders are assigned penalties in proportion to their worth and not their difficulty.
