You could have a secret subway station to ship supplies in and out, as well as to deliver certain criminals to more secure facilities. Similarly, doctors and nurses could be brought to the infirmary by a secure and secret path. And those the infirmary can'i handle can goto the hospital also by a secure and secret path.
