As for "Runaround" Attacks (p. B391): Turn to p. B389, look at the diagram, and imagine that your foe is the figure in the hex with the arrow. You can only achieve a true attack from behind if you start your turn in one of the black hexes. If you start in one of the gray or white hexes and move into the black hexes, it's merely a runaround. Thus, if you're adjacent to your foe at the start of your turn, there's just one hex that will count for a true attack from the rear, and that's the black one directly behind him.
And your turn starts when you get to choose a maneuver, not before. Your defenses immediately before you take your next maneuver are part of last turn, not this turn.
